INTRODUCTION AND IMPORTANCE: This study aimed to determine the impact of DM, HTN and age on IVC dimensions as measured by CT scan relevant to guide interventions in a Jordanian population. PRESENTATION OF CASES: Two hundred patients were selected from those referred to the Radiology Department, Jordan University Hospital, Amman, Jordan for clinical evaluation. Patients were divided into three age subgroups. Age, sex, and comorbidities such as DM and HTN were identified and saved for later use. All dimensions of the IVC were measured using an abdomino-pelvic CT scanner. CLINICAL DISCUSSION: A full morphometric analysis of the IVC would provide a better understanding of the dynamicity of the IVC in relation to its blood flow. Our results revealed that the length of the IVC was significantly shorter with age (P = 0.003). DM significantly affected the length of the IVC (P = 0.044). Hypertension also significantly affected the length of the IVC (P = 0.031), but it did not significantly affect the anterio-posterior or the transverse diameters of the IVC. CONCLUSION: The length of the IVC was significantly shorter with age, DM and hypertension. Morphometric measures of the IVC are of great clinical importance as they may assist in medical or surgical intervention and follow-up.

BACKGROUND: Dromedary or one-humped camel (Camelus dromedarius) is distinctively acclimatized to survive the arid conditions of the desert environment. It has an excellent ability to compete dehydration with substantial tolerance for rapid dehydration. Therefore, it offers an excellent model for studying osmoregulation. Molecular characterization of Na+/K+ ATPase as a central regulator of electrolyte normohemostasis affords a better understanding of this mechanism in camel. Here is the first to resolve the full-length of alpha-1 subunit of sodium pump (ATP1A1) gene with its differential expression in dromedary tissues. RESULTS: The nucleotide sequence for the recovered full cDNA of ATP1A1was submitted to the GenBank (NCBI GenBank accession #MW628635) and bioinformatically analyzed. The cDNA sequence was of 3760 bp length with an open reading frame (ORF) of 3066 bp encoding a putative 1021 amino acids polypeptide with a molecular mass of 112696 Da. Blast search analysis revealed the shared high similarity of dromedary ATP1A1gene with other known ATP1A1genes in different species. The comparative analysis of its protein sequence confirmed the high identity with other mammalian ATP1A1 proteins. Further transcriptomic investigation for different organs was performed by real-time PCR to compare its level of expression among different organs. The results confirm a direct function between the ATP1A1 gene expression and the order of vital performance of these organs. The expression of ATP1A1 mRNA in the adrenal gland and brain was significantly higher than that in the other organs. The noticed down expression in camel kidney concomitant with overexpression in the adrenal cortex might interpret how dromedary expels access sodium without water loss with relative high ability to restrain mineralocorticoid-induced sodium retention on drinking salty water. CONCLUSION: The results reflect the importance of sodium pump in these organs. Na+/K+ ATPase in the adrenal gland and brain than other organs.

The study presented an extremely rare case of real complete bilateral duplication of inferior vena cava (IVC) in a male cadaver which has never been reported before. Both IVC had approximately the same diameter. The right IVC drained into the right atrium; the left IVC continued as hemiazygos vein and drained into the superior vena cava. Three anastomotic venous channels, a cranial preaortic, a middle and a caudal retroaortic, joined both vessels. Multiple variations in the way of drainage of posterior intercostal veins, on both sides, were also present. The present report invalidates an old classification defining the two vessels when joined at the level of the renal veins as complete bilateral duplication of IVC. Although the presence of combination of venous variations is extremely rare, awareness of such variations is essential for clinical and surgical procedures to avoid misdiagnosis and surgical complications.

The main objective of the current study was to assess the impact of the water taken from the 'Tunisian Refining Industries Company' on meiobenthic nematodes, before and after a series of treatments in decantation basins followed by its discharge in Bizerte bay, Tunisia. The comparison of environmental parameters of the two types of water was clearly indicative of an improvement in the quality of treated waters after a significant reduction in their loads in hydrocarbons. Overall, the water retained a good quality after being treated by 'Tunisian Refining Industries Company' before discharge in the sea. At the end of the experiment, differential responses were observed according to the richness of sediment in organic matter and hydrocarbons. Thus, it was apparent that the nematode assemblage exposed to the treated waters was closer to controls and associated to higher values of abundance, than that under untreated ones. It was also assumed that the species Microlaimus honestus De Man, 1922, Paramonohystera proteus Wieser, 1956 and Cyartonema germanicum Juario, 1972 are sensitive bioindicators of bad environmental statues and of hydrocarbon presence in the environment. On the other hand, Metoncholaimus pristiurus (Zur Strassen, 1894) Filipjev, 1918 would rather be classified as a positive bioindicative species of this type of pollutants.


O principal objetivo do presente estudo foi avaliar o impacto da água retirada da "Tunisian Refining Industries Company" sobre os nematoides meiobentônicos, antes e depois de uma série de tratamentos em bacias de decantação seguidos de seu descarte na baía de Bizerte, Tunísia. A comparação dos parâmetros ambientais dos dois tipos de água foi claramente indicativa de uma melhoria na qualidade das águas tratadas após uma redução significativa das suas cargas em hidrocarbonetos. No geral, a água reteve uma boa qualidade após ser tratada pela "Tunisian Refining Industries Company" antes da descarga no mar. Ao final do experimento, foram observadas respostas diferenciais de acordo com a riqueza de sedimentos em matéria orgânica e hidrocarbonetos. Assim, ficou claro que a assembleia de nematoides exposta às águas tratadas estava mais próxima dos controles e associada a maiores valores de abundância do que nas não tratadas. Também foi assumido que as espécies Microlaimus honestus De Man, 1922, Paramonohystera proteus Wieser, 1956 e Cyartonema germanicum Juario, 1972 são bioindicadores sensíveis de más estátuas ambientais e da presença de hidrocarbonetos no meio ambiente. Por outro lado, Metoncholaimus pristiurus (Zur Strassen, 1894) Filipjev, 1918 preferiria ser classificado como uma espécie bioindicativa positiva deste tipo de poluentes.

The organization of the roots, ganglia and the peripheral distribution of the cranial nerves of the fully formed embryos of Oreochromis niloticus are examined in the transverse serial sections. These nerves carry fibers, which were also analyzed. The results of this study demonstrated that the glossopharyngeal nerve originates by means of only one root, which leaves the cranium through the glossopharyngeal foramen. This nerve gives fibers (visceromotor) to the first internal and external levator arcus branchialis muscles. There is a single epibranchial (petrosal) ganglion located extracranially. Nervus glossopharyngeus has three rami; pharyngeus, pretramticus and posttrematicus. The ramus pharyngeus carries only viscerosensory fibers; general for the pharyngeal epithelium and special ones for the pseudobranch. General viscerosensory fibers are also carried by rami pretrematicus and posttrematicus for the pharyngeal epithelial lining. The special sensory fibers are carried by the ramus pretrematicus for the taste buds and by ramus posttrematicus for the gill filaments. The ramus pretrematicus also carries visceromotor fibers for the first adductor arcus branchialis and to the first obliquus ventralis muscles.


A organização das raízes, gânglios e a distribuição periférica dos nervos cranianos dos embriões totalmente formados de Oreochromis niloticus são examinados nas seções transversais seriais. Esses nervos carregam fibras, que também foram analisadas. Os resultados deste estudo demonstraram que o nervo glossofaríngeo se origina por meio de apenas uma raiz, que sai do crânio pelo forame glossofaríngeo. Este nervo fornece fibras (visceromotoras) para os primeiros músculos levantadores do arco branquial interno e externo. Existe um único gânglio epibranquial (petroso) localizado extracranialmente. Nervus glossopharyngeus tem três ramos; faríngeo, pretramticus e póstrematicus. O ramo faríngeo contém apenas fibras viscerossensoriais ­ gerais para o epitélio faríngeo e especiais para o pseudobrânquio. Fibras viscerossensoriais gerais também são transportadas por ramos pretrematicus e posttrematicus para o revestimento epitelial da faringe. As fibras sensoriais especiais são transportadas pelo ramus pretrematicus para as papilas gustativas e pelo ramus posttrematicus para os filamentos branquiais. O ramo pretrematicus também carrega fibras visceromotoras para o primeiro adutor arcus branchialis e para o primeiro músculo oblíquo ventral.

OBJECTIVES: Intraoperative identification of anatomical structures can potentially reduce the risk of surgical complications. This study aims to report specialists' perspectives about the anatomical structures that third-year residents should be able to identify during surgical operations. In addition, the factors which may influence specialists' opinions are discussed. MATERIALS AND METHODS: This qualitative cross-sectional study was conducted on obstetricians and gynaecologists between 1/2/2019 and 30/10/2019. The specialists practising in a hospital with a residency programme were included, and were asked to rate the importance of structures that a third-year resident should be able to identify during operations. We performed a comparison of responses based on specialists' age, gender, practice type, years of experience, and surgical workload. RESULTS: One hundred and sixty-five specialists were recruited with a response rate of 69.3%. The mean age of respondents was 46.1 years, and they had a mean experience of 13.4 years. Furthermore, 86.6% of specialists rated all the anatomical structures as "more important". The importance of surgical structures, as rated by specialists, was not related to gender, years of experience, or surgical workload. The importance of 63% of the anatomical structures was rated higher by junior specialists than senior specialists. CONCLUSION: Knowledge of anatomical structures is vital for gynaecologic residency training. Specialist's perceptions of the importance of various anatomical structures reflect their understanding of the training requirements. Our results highlighted the important anatomical structures that third-year residents are expected to identify during surgical operations. Future research may establish a reference for the core anatomy knowledge essential for each training year.

Appropriate treatment of bladder cancer (BC) is widely based on accurate and early BC staging. In this paper, a multiparametric computer-aided diagnostic (MP-CAD) system is developed to differentiate between BC staging, especially T1 and T2 stages, using T2-weighted (T2W) magnetic resonance imaging (MRI) and diffusion-weighted (DW) MRI. Our framework starts with the segmentation of the bladder wall (BW) and localization of the whole BC volume (Vt) and its extent inside the wall (Vw). Our segmentation framework is based on a fully connected convolution neural network (CNN) and utilized an adaptive shape model followed by estimating a set of functional, texture, and morphological features. The functional features are derived from the cumulative distribution function (CDF) of the apparent diffusion coefficient. Texture features are radiomic features estimated from T2W-MRI, and morphological features are used to describe the tumors' geometric. Due to the significant texture difference between the wall and bladder lumen cells, Vt is parcelled into a set of nested equidistance surfaces (i.e., iso-surfaces). Finally, features are estimated for individual iso-surfaces, which are then augmented and used to train and test machine learning (ML) classifier based on neural networks. The system has been evaluated using 42 data sets, and a leave-one-subject-out approach is employed. The overall accuracy, sensitivity, specificity, and area under the receiver operating characteristics (ROC) curve (AUC) are 95.24%, 95.24%, 95.24%, and 0.9864, respectively. The advantage of fusion multiparametric iso-features is highlighted by comparing the diagnostic accuracy of individual MRI modality, which is confirmed by the ROC analysis. Moreover, the accuracy of our pipeline is compared against other statistical ML classifiers (i.e., random forest (RF) and support vector machine (SVM)). Our CAD system is also compared with other techniques (e.g., end-to-end convolution neural networks (i.e., ResNet50).

SUMMARY: This study aims to evaluate the ability of resveratrol (RSV) antioxidant to attenuate the oxidative stress condition induced by secondhand exposure of cigarettes and waterpipe smoking using animal model. Forty-eight mice were divided equally into six different groups, and RSV was delivered to certain groups intraperitoneally with a dose of 25 mg/kg/day. The process of smoking exposure was performed using a specialized smoking machine. The experiment duration lasts for six consecutive weeks. Five µm sections of lung were stained with hematoxylin and eosin for light microscopy, and 70 nm ultrathin sections of lung stained with uranyl acetate and lead citrate were prepared for transmission electron microscopy to observe the cellular ultrastructure. In lung, RSV reduced the deterioration and blebbing of bronchiole epithelium, reduced the inflammation, increased the surface area of alveolar sac, and reduced the thickening of alveolar walls. Also, blood vessels were less congested and less dilated with less diffusion of extravasated blood. Ultrastructural images illustrated that RSV protects the normal structure of alveolar septum, prevents DNA damage and alveolar degeneration, showed less degree of apical membrane blebbing and retained the uniform pattern of mitochondria. In conclusion, RSV has ameliorative effects against the oxidative stress condition induced by secondhand (side stream) exposure of cigarette and waterpipe tobacco smoking.


RESUMEN: Este estudio tiene como objetivo evaluar la capacidad del antioxidante resveratrol (RSV) para atenuar el estrés oxidativo inducido por la exposición de segunda mano a los cigarrillos y al tabaquismo en pipa de agua, utilizando un modelo animal. Se dividieron cuarenta y ocho ratones en seis grupos diferentes, y se administró RSV a ciertos grupos por vía intraperitoneal con una dosis de 25 mg / kg / día. El proceso de exposición al tabaquismo se realizó utilizando una máquina de fumar especializada. La duración del experimento fue de seis semanas consecutivas. Para realizar la microscopía óptica se tiñeron secciones de pulmón de cinco µm con hematoxilina y eosina, y se prepararon secciones ultrafinas de 70 nm de pulmón teñidas con acetato de uranilo y citrato de plomo para la microscopía electrónica de transmisión para observar la ultra estructura celular. En el pulmón, el RSV redujo el deterioro y la formación de ampollas del epitelio de los bronquiolos, redujo la inflamación, aumentó la superficie del saco alveolar y redujo el engrosamiento de las paredes alveolares. Además, los vasos sanguíneos se encontraron menos congestionados y menos dilatados y con menor difusión de sangre extravasada. Las imágenes ultraestructurales mostraron que el RSV protege la estructura normal del tabique alveolar, previene el daño del ADN y la degeneración alveolar, mostrando un menor grado de formación de ampollas en la membrana apical y además retuvo el patrón uniforme de las mitocondrias. En conclusión, el RSV tiene efectos de mejora contra el estrés oxidativo inducido por la exposición de segunda mano (corriente secundaria) al fumar cigarrillos y pipas de agua.

BACKGROUND: Hepatitis B and C viruses are leading causes of liver cirrhosis and hepatocellular carcinoma (HCC). Toll-like receptor 7 (TLR-7) has been implicated in the pathogenesis of HCC linked to hepatitis B. We hypothesised a role of leukocyte TLR-7 mRNA in hepatitis C related liver cirrhosis and HCC, using alpha-fetoprotein (AFP) and liver function tests as comparators. METHODS: We recruited 102 patients with HCV-related HCC, 97 with HCV-related liver cirrhosis and 60 healthy controls. Quantification of TLR-7 mRNA was performed using real-time PCR, AFP and routine LFTs by standard techniques. RESULTS: TLR-7 mRNA levels were significantly lower in HCC patients compared to cirrhotic patients and lower again in healthy controls (p < 0.001 for trend). In multivariate analysis, age, aspartate transaminase (AST), AFP, and TLR-7 mRNA were significant predictors of HCC. The ROCC/AUC for age, AST and TLR-7 mRNA were all between 0.64 and 0.78 (all P < 0.01), but for AFP was 0.57 (95% CI 0.48-0.65, P = 0.09). We derived an index score using age, AST and TLR-7 mRNA for the diagnosis of HCC. The ROCC/AUC for the index was superior to all three root indices in the prediction of HCC. The index linked significantly with the Tokyo and Vienna liver cancer staging systems, but not with those of the CLIP and Okuda systems, in distinguishing HCC from liver cirrhosis. CONCLUSION: The combination of TLR-7 mRNA levels with age and AST improves the performance of TLR-7 in HCC diagnosis, out-performs alpha-fetoprotein and predicts early HCC.

The study of anatomy is essential in understanding basic and clinical medical sciences and it is taught by various methods. This study aimed to find out the attitude of medical students in Jordan towards the various methods of teaching anatomy and their opinion regarding the relevance of anatomy in their study. An online survey was sent to the third-to sixth-year medical students at the Hashemite University, Jordan. The survey asked about various aspect of teaching anatomy. The answers were analyzed using non-parametric methods.The response rates ranged from 24% to 45% in the different groups. More than half the students believed that anatomy was interesting, but only a small percentage thought about becoming anatomists. In all the groups, plastic models were considered the most favored method of teaching and theoretical lectures were the least favored. The students preferred the number of lab session to be increased and lectures to be decreased. Text-books were not considered as a main source of study. A very high percentage of students in all the groups considered digital media as a main source of study preferring it to be incorporated more in the curriculum. A greater percentage of students in the third and fourth years thought that too much information was given in their anatomy courses. The greater percentage of students in the fifth and sixth years did not. Almost all the students believed that anatomy was important in understanding basic and clinical medical sciences and in their future profession as doctors. Modifications in the anatomy curriculum may be required. More practical sessions should be arranged, reading textbooks should be encouraged, and new technology and digital media need to be incorporated more

SUMMARY: Postgraduate refresher courses may address deficiencies in the gross anatomy preparedness of medical graduates. However, the literature does not offer a method to identify such deficiencies. Our aim is to develop and validate a scale to measure the gross anatomy preparedness of medical graduates. First, we defined gross anatomy preparedness (the construct) as "the benchmark of personal ability in gross anatomy against the standard required for clinical practice." Next, we conducted a literature search for extant items related to our definition. To develop our scale, we grouped the items under three headings: proficiency, preference, and pertinence. Finally, we constructed item-specific response anchors to "Likertize" the items. We recruited experts to validate the content and conducted cognitive interviews to validate the response process. To evaluate the internal structure and reliability of the scale, we invited a purposive sample of 120 surgery residents to complete the scale and explored the results of the pilot test using data reduction and reliability analysis. A total of 77 surgery residents completed the scale. Varimax-rotated principal components analysis revealed three components with eigenvalues greater than one, and the components explained 64 % of the total variance. The rotated solution was consistent with the original structure of the questionnaire. The components, which represented the proficiency, preference, and pertinence item sets, explained 25 %, 23 %, and 16 %, respectively, of the total variance. Cronbach's α coefficients for the item sets were 0.72, 0.71, and 0.61, respectively. We developed and validated a scale to measure the gross anatomy preparedness of medical graduates. In addition, we offer conceptual guidelines to help users interpret the results of the scale. Outcome data are required to substantiate the predictive validity of the scale.


RESUMEN: Los cursos de actualización de posgrado pueden abordar las deficiencias en la preparación de la anatomía macroscópica de los graduados médicos. Sin embargo, la literatura no ofrece un método para identificar tales deficiencias. Nuestro objetivo fue desarrollar y validar una escala para medir la preparación anatómica general de los graduados médicos. Primero, definimos la preparación para la anatomía macroscópica (el constructo) como "el punto de referencia de la capacidad personal en anatomía macroscópica frente al estándar requerido para la práctica clínica". A continuación, realizamos una búsqueda bibliográfica de elementos existentes relacionados con nuestra definición. Para desarrollar nuestra escala, agrupamos los ítems bajo tres encabezados: competencia, preferencia y pertinencia. Finalmente, construimos anclas de respuesta específicas del ítem para "dar me gusta" a los ítems. Our aim was to investigate the variation in the vertebral levels of the origins of the celiac artery, superior and inferior mesenteric arteries, paired renal arteries, and common iliac arteries. We conducted a retrospective imaging study in a large public secondary hospital on a nonrandom sample of 227 participants. We consecutively included adult patients who had undergone computed tomography angiography of the abdomen and excluded patients with a history of any vertebral abnormality or whose images revealed evidence of a vertebral abnormality or a congenital anomaly of any of the branches of the abdominal aorta. The primary outcome was the frequency distribution of the vertebral levels of the landmarks. The secondary outcomes were the intercorrelations of the vertebral levels of the landmarks and their relationships with age, sex, weight, height, and body mass index. The celiac artery originated at T11/T12-L1/L2, followed by the superior mesenteric artery at T12-L2, the paired renal arteries at T12/L1-L2/L3, the inferior mesenteric artery at L2-L4, and the common iliac arteries at L3-L5. The vertebral levels of the landmarks were positively intercorrelated and stronger between proximate pairs. In addition, the vertebral levels of the landmarks were related to age, but not sex, weight, height, or body mass index. The intercorrelations suggest that a considerable proportion of the variation is accounted for by 'trickle-down' variation; variation in the vertebral level of a proximal landmark results in variation in the vertebral level of the immediate distal landmark. The overarching parameter remains unidentified.

INTRODUCTION: Early detection of breast cancer is important in diagnosis and treatment, and so in enhancing patient survival and reducing death rates. Because of the low diagnostic sensitivity and specificity of widely used breast cancer biomarkers such as CA15-3, we hypothesised that a panel of new metabolic markers would provide superior sensitivity and specificity for this disease. MATERIAL & METHODS: We recruited 120 women with malignant breast cancer, 47 with benign breast disease and 55 females as a healthy control group. Metabolites 8-hydroxy-2'-deoxyguanosine, 1-methylguanosine, and 1-methyl adenosine were detected and quantified by gas chromatography-mass spectrometry, CA15.3 by ELISA. Cut-off values of individual and combined metabolome with CA15-3 were analysed using the receiver operating characteristics curve (ROCC) to test the efficiency of the candidate metabolome in identifying breast cancer. RESULTS: The overall linear trend of biomarkers across the groups was significant with highest levels in breast cancer (all p < 0.05). Using cut-off values of CA15-3, 8-hydroxy-2'-deoxyguanosine, 1-methylguanosine and 1-methyl adenosine of 30.5 U/l, 15.0 µg/l, 18.5 µg/l and 22.0 µg/l, respectively, diagnostic performance analyses of combined metabolome with CA15-3 gave a ROCC area under the curve of 0.94 (95% CI: 0.91-0.98)(p < 0.01) with good sensitivity (88.8%), specificity (86.8%) and efficiency (90.6%). Unlike CA15.3, the highest levels of each of the metabolite were in the early stage of breast cancer. CONCLUSION: The diagnostic combination test of candidate metabolome with CA15.3 may be a useful tool for the early detection of breast cancer and used as a metabolomics signature in this disease.

Cardiac magnetic resonance (MR) imaging is one of the most rigorous form of imaging to assess cardiac function in vivo. Strain analysis allows comprehensive assessment of diastolic myocardial function, which is not indicated by measuring systolic functional parameters using with a normal cine imaging module. Due to the small heart size in mice, it is not possible to perform proper tagged imaging to assess strain. Here, we developed a novel deep learning approach for automated quantification of strain from cardiac cine MR images. Our framework starts by an accurate localization of the LV blood pool center-point using a fully convolutional neural network (FCN) architecture. Then, a region of interest (ROI) that contains the LV is extracted from all heart sections. The extracted ROIs are used for the segmentation of the LV cavity and myocardium via a novel FCN architecture. For strain analysis, we developed a Laplace-based approach to track the LV wall points by solving the Laplace equation between the LV contours of each two successive image frames over the cardiac cycle. Following tracking, the strain estimation is performed using the Lagrangian-based approach. This new automated system for strain analysis was validated by comparing the outcome of these analysis with the tagged MR images from the same mice. There were no significant differences between the strain data obtained from our algorithm using cine compared to tagged MR imaging. Furthermore, we demonstrated that our new algorithm can determine the strain differences between normal and diseased hearts.

Anatomists and radiologists use the Zaidi-Dayal and Richards-Jabbour scales to study the shape of the foramen magnum. Our aim is to measure the interrater and intrarater agreement and reliability of ratings made using the two scales. We invited 16 radiology residents to attend two sessions, four weeks apart. During each session, we asked the residents to classify the shape of the foramen magnum in 35 images using both scales. We used Fleiss' κ to measure interrater reliability and Cohen's κ to measure intrarater reliability. The interrater reliability of ratings made using the Zaidi-Dayal scale was 0.34 (0.26-0.46) for session one and 0.30 (0.24-0.39) for session two, and the intrarater reliability was 0.39 (0.34-0.44). The interrater reliability of ratings made using the Richards-Jabbour scale was 0.14 (0.10-0.19) for session one and 0.12 (0.09-0.17) for session two, and the intrarater reliability was 0.11 (0.07-0.15). In conclusion, the interrater and intrarater agreement and reliability of ratings made using the Zaidi-Dayal and Richards-Jabbour scales are inadequate. We recommend an objective method by Zdilla et al. to researchers interested in studying the shape of the foramen magnum.
